Sorry not sure what you are asking. Do you want to know how much maintance a 2003 Exchange server requires? Or are you having issues with your Exchange server.

Microsoft System Center Operations Manager has some proactive trending that it does.  There are also other products/services like Zenprise that will provide proactive monitoring.

The biggest thing to do is to get your server configured correctly.  If you configure it correctly there should be very little that you need to do manually.

  1. Make sure your server is properly sized for your client load.
  2. Make sure your online database maintenance is running and completing on each database weekly.
  3. Make sure you are running online backups outside of the database maintenance window.
  4. Install and cofigure an Exchange aware Anti-virus that automatically updates definitions.
  5. Be sure to patch your Exchange server with all critical applicable security updates.
  6. Run the Exchange Best Practices Analyzer (ExBPA) and follow up on any issues.
  7. Make sure to monitor the server using a monitoring tool like System Center Operations Manager (or Essentials).

After doing these things you should have a pretty pain free time.
